The physician documented "necrotic sutures all around the flap which caused it to dehisce and fail". The physician noted as chief compliant "Failed muscle flap of sacrum (T84.401A)." The physician then noted in the assessment "Dehiscence of closure of muscle flap" (T81.31XA). Apparently, this is the primary reason for admission but no treatment plan was identified. Can someone please assist with choosing the correct principal dx code based on the two listed above? Assigning the wrong principal dx code in this case could prove to have a huge financial impact on the patient's assigned DRG. Would both codes be assigned?
