If a doctor states in the EHR that at pt has visual disturbance and restless leg syndrome but does not put that down as their dx and only list their chronic conditions can I myself use what he states in the chart and code that? Thanks!
 
What are the chronic conditions listed?

If I understand your post correctly, as long as the doctor put in the EHR that the patient has visual disturbance and restless leg syndrome you can code it.
