Due to the value modifier, what is the automatic payment penalty percentage that physicians will incur on all MPFS allowed charges in 2017 if they do not participate in the PQRS in 2015?


Requesting elaborate info on below,

By claim based reporting, if physician participating in 5 PQRS Measures. Doucmentation criteria met for 75% of reports in 3 measures and completely not met for other 2 measures, please provide idea whether penalty would be applicable on this case.
