Wiki Fee schedule for preventive medicine

We are a general surgery practice, and may be hiring an internal med. doctor. I was looking into the reimbursement for preventive medicine codes 99381-99387 and 99391-99397. Usually we set our fees a percentage above Medicare's fee schedule, but since Medicare doesn't allow coverage for these codes I'm not sure how to calculate our fee. Anyone want to share any info, or point me in the right direction on determining the fees.

P.S. I have my Blue Cross Blue Shield conversion factor, but again it's multiplied by Medicare's fee, which doesn't exsist....
