Does anyone bill Fiducial Markers in office for the prostate cancer? Do you bill 55876, 76942, and A4648? What does Medicare reimburse?

We bill 55876, 76965 & A4648. We haven't done one in a while so I'm not sure what Medicare reimbursement is. A lot of times they will request the invoice to be sent with the claim.

We bill these in the office often. We bill 55876, 76872, A4648. We have to send the invoice for the markers and reimbursement is roughly $265.00 total

my doc places 5 markers; I billed A4648 x 5 as the HCPC description states, each; some privates have paid but issue with Medicare. anyone else?

We get the A4648 Implantable Tissue Markers in packs of 3. Does anyone know if we can bill waste for the ones not used and have to be trashed?
