Does anyone know where to find documentation to show that each Radiology report must stand alone and each diagnosis coded must come from that particular report? I have a radiologist that wants to use a diagnosis found on another report done on the same date of service. However it was my understanding that if that finding, or symptom was not dictated on that particular procedure report then it could not be coded. Is this true? Or can I take a symptom or finding from a CT of the Head and apply it to an MRA of the Head if it was done on the same date of service?
