Would someone please look at this operative report and tell me what CPTs you would use? I'm looking at 11750 and maybe 26951. I'm not sure if this is enough for a partial amputation. If not amputation, then maybe 10060?

I would go with 11750. Note to me reads that between the proximal and distal nail plate is where mark is. I believe that when the matrix was ablated the bone had to be rongeured for closure.
