Full thickness excision of helical rim is performed leaving at least 5 mm margins leaving a 3 cm defect in the helical rim. Star-shaped full thickness (skin and cartilage) is performed to create advancement composite flaps. Cartilage is trimmed. Flaps are advanced and cartilage is approximated using 4-0 PDS on taper needle. Skin is closed with interrupted 6-0 nylon. Frozen section done after composite excision. Shave biopsy of left antitragus also done in same surgery.
