Flex Sig post total colectomy

I need help with both CPT and Dx codes for procedure on patient that had a total colectomy with a pull through to attach to rectum several years ago. Scope was advanced through the rectum and the anastomosis was approx. 15 cm from the anal verge, and advanced to 25 cm, including the neoterminal ileum. Biopsies were done in the ileum and in the surgically created rectal pouch. Path findings read "patchy chronic ileitis" and "chronic colitis" respectively. How do I code both the procedure and the Dx? Thank you in advance.
