Is there a way to get paid for seeing a patient that has not been in office in over 6 months
but has no conditions going on today but wanting to get their flu shot. Our office wants
to examine patient before giving them shot. Is there a right way to code this and get paid
for it? Or just charge for the flu shot and administration?

If they perform an age/sex appropriate history and exam, and counsel the patient on any identified risk factors relevant to their age/sex/lifestyle, then you could report a preventive exam. Absent any problems, you can't report 99211-99215. If their entire purpose in coming in was to get a flu shot, and they didn't request a PE, I'd be careful about billing one - they may not have coverage for it, and probably wouldn't be too happy about getting a bill for it, when they could have just gone to a pharmacy and gotten a flu shot.

If by "examine them", you mean "get their vitals and make sure that they're not allergic to eggs", don't report an E/M - that much is included in the pre-procedure work for the vaccine admin code. ;)
