We were watching a webinar today where to instructer advised we bill a well visit with a flu shot as follows

99382 V20.2
90685 V20.2
90460 V20.2

In the past we have not linked the well dx to the flu shot. Has anyone had any experience billing like this? Does anyone have any other documentation to back this up?

You could also use v04.81 as Dx code for Flu vaccine and administration instead of v2.02. There should also be a 25 modifier on the well visit.
 
I have been billing well visits with flu vaccines, both IM and FluMist, for the past few weeks with V20.2 on all lines and I have not had any problems with reimbursement. I was a little gun shy at first and only did a handful to each payer to see how they would be processed, but it seems they are processing flu vaccines the same way as other immunizations with a well child visit.

Use of code V20.2 for immunizations at the well child visit is appropriate per the inclusion notes found at code V20.2. "Immunizations appropriate for age" is one of the inclusion terms.

The AAP "Bright Futures Tool and Resource Kit" advises to use the well child diagnoses V20.31, V20.32, and V20.2 on all codes used with well child visits, as long as there is an actual well child check. If an E/M code for an office visit is coded instead of a well visit (99201-99205, 99211-99215) then use would code the individual vaccine diagnosis code.

The example the give:
99393-25, V20.2
90633, V20.2
90700, V20.2
90660, V20.2
90460 x 3, V20.2
90461 x 2, V20.2

We have been doing this for about 3 years now and have not had any problems with reimbursement. It is so much easier to post charges, as well!
