I am tried to code, but not geting the correct CPT codes , so please help me to do this... anybody ...... thanks....

Diag.... ESRD on regular hemodialysis

Procedure :Fluroscopy guided,RT femoral access+SVC venogram+PTA

Access through the RT femoral vein, SVC venogram showed two stenosis in the SVC.10x40mm and 12x60mm baloon were used to dilate the stenosis. Post dilatation venogram showed good results and no complications.SVC stent was not inserted due to the presence of the central dialysis catheter.
