For anyone that codes out for physician or non physician filling out FMLA or disability do you code out 99080 or 99455? Thanks.
 
99455 is for an examination, so definitely not that. 99080 would be more accurate for filling out paperwork. Or you can create your own internal code for this, since it is not being billed to insurance anyway. As a practice, since we're gynecologic oncology, decided our patients have enough to worry about and we do not charge the patient and do not code it. Many other specialties in my organization do charge.
