Please help!!!

So this is the scenario:

For a RUL bronchial washings, RUL bronchial brush/washing, RUL lung biopsy, right paratracheal fna, subcarinal fna, right hilum fna, cytologies: 1. squamous cell carcinoma, grade 3 of 3.

Intraoperative Consultation:

Fast stain Interpretation:
(Paratracheal) (Subcarinal) (Right Hilar) all 3 have their own documentation.

Gross and Microscopic Description also documented.

What would be the correct procedure codes?
It was listed with the 88112 88177 88305 and 88173 but can 88177 be used without the 88172???
 
+88177 is an add on code to 88172- per site--
You mentioned three sites Paratracheal, Subcarinal,Right Hilar, were adequacy checks perfomed on all three..? Then codes are 88172x3 & 88173x3.

88177 is for the additional passes so therefore the x2 would go on line 88177 not 88172 or 88173. this is why the rule was created. the addt'l passes will not be posted to 88172 or 73
Ex: 3 passes would bill as
88172
88173
88177 (x2)
 
If a cell block was made, then 88305 is an acceptable code in addition to the others (I haven't checked them, my code book isn't nearby and that seems to have been addressed above). A cell block carries its own code for its examination and diagnosis, Cell block, any source (88305).

Since the sites are all different then I believe the correct coding would be
88172 x 3
88173 x 3
as Indy29 stated. The 88177 would be if the same site were repeated because the pass was inadequate and a second pass was made (and documented). That's the way it's noted in my CPT book below 88177.
