patient sees primary OB to check on follicles as trying to conceive natrually. Dr performes transvaginal ultrasound. is correct CPT 76830 or 76857? also which ICD-10 code is to use?

The description of 76830 and 76857 should clearly lead you to the correct code as you state it is a "transvaginal ultrasound."
76830 Ultrasound, transvaginal
76857 Ultrasound, pelvic (nonobstetric), real time with image documentation; limited or follow-up

Regarding the ICD-10, you would code whatever reason the specific patient is having the scan, or findings of the scan.
There are literally thousands of possible reasons to perform an ultrasound. Possible examples:
N83.11 for right corpus luteum cyst
N97.1 for female infertility, tubal origin
R10.2 for pelvic pain
Z31.69 for encounter for other general counseling and advice on procreation
