My question is a very basic question but I seem to keep confusing myself. A patient was seen for ankle sprain and sent to PT and given prescription. I coded the acute sprain. 4wks later the patient returns for follow-up and doctor now documents sprain is resolved but contusion of ankle still present, do I use the orothopedic f/u DX code or do I code the contusion? At what point do I stop coding the acute code and code a/c?
