I have a patient who has Fordyce spots on his scrotum. Is it appropriate to use Q38.6 for this although ICD-10 only lists this dx for the mouth?

No, this diagnosis would not meet the definition of Q38.6 which is a 'congenital malformation of the mouth'. I would recommend L73.8 as more appropriate for this.
