If a patient comes in with a superficial foreign body in the cornea or conjunctiva what has to happen for 65220 or 65205 to be charged? If a q-tip is used to remove the foreign body can we charge one of these or does a needle have to be used in order to charge?
 
In the Coders' Desk Reference, part of the description for both 65205,65220 states "an incision may be required to remove an embedded foreign body". I take it as may or may not. Would like to hear other opinions.
