I need some help ... patient doesn't want to file charges to health insurance, is there a form or waiver that is needed to cover us?

Yes, there is a form. I am not exactly sure where my boss found it, but it says Request for Restrictions on Use/Disclosure of PHI to a Health Plan for Payment and Health Care Operations at the top of the page. Try Google, you can find anything. Hope this helps you.
