Anyone billing for TCM? Are you billing an EM level with the transition care code (99495 or 99496)?

The TCM has the face to face component inside the code, so I would not bill an additional E/M level.

A FACE-TO-FACE VISIT
You must furnish one face-to-face visit within certain timeframes as described by the
following two Current Procedural Terminology (CPT) codes:
™™ CPT Code 99495 – Transitional care management services with moderate medical
decision complexity (face-to-face visit within 14 days of discharge)
™™ CPT Code 99496 – Transitional care management services with high medical
decision complexity (face-to-face visit within 7 days of discharge)
The face-to-face visit is part of the TCM service, and you should not report it separately.

FQHC biller too!

Would TCM be appropriate to use if a patient is discharged from an inpatient behavioral health facility? This location is not listed in the CPT book. I would appreciate any input.
