Wiki FQHC billing fingerstick by NP/PA

I work for an FQHC. What is the proper way to bill for a fingerstick (PT/INR) ordered by a NP but the fingerstick was done by a nurse (LPN). The NP visit and fingerstick were done the same day but each at different office locations. The supervising physician works at both locations and supervises both the NP and LPN. If anyone has suggestions, I would appreciate your help.
