I am new to the FQHC billing world and I am still learning. There has been a question within my facility on how we bill any MA plan when there is a commercial plan as Primary. Obviously with the commercial plan, there is no T-code or G-code associated with the original claim. We have learned that most of these secondary claims will need to be sent as paper claims with the EOB from the primary and we will need to add the T-code to the secondary claim. My question is....what charge do we put on the T-code for the secondary claim? Do we put what is actually due from the primary that is showing as patient responsibility or do we put our PPS rate? I have a hard time trying to fathom putting a $106.89 PPS rate on a secondary claim when the total responsibility is only a $25 copay. Thank you for your help.
