Looking for information on the appropriate code for the fresnel prism. Under what circumstances will insurance pay on this, if they will pay what are the covered diagnosis codes?
To my knowledge, most major medical plans, with the exception of the limited Medicare coverage for glasses, do not cover optical devices. Therefore, the patient should pay out of pocket. Those are fairly inexpensive so it really shouldn't be a major problem for the patient to pay out of pocket IMHO.
