Has anyone else gotten denials for use of FX modifier for Medicare claim on x-rays? We used the FX modifier on 74000 because the practice still use film for x-ray. Which caused a denial for use of inappropriate modifier. When we call Medicare we reference Change Request CR 9727 and MLN Matters Number MM9727.
 
One of our providers is still using film ... and we've reported FX without denial (in Florida, though). Maybe your MAC just hasn't updated their system? Of course mistakes happen. But, it they wait too long to correct it, don't hesitate to reach out to the CMS Regional Office for the area.
