please help me understand on how to bill G2211? and who can bill it. i am kind of confuse on this cpt code.

CMS has several articles on how this code is to be used. Check the website or your MAC. It's a HCPCSII code, so CPT won't have any information.

PRS Network has a few good podcasts explaining its usage. It's an add-on code to an E&M when the provider sees a patient with a single, ongoing, complex medical condition. It can't be billed when a procedure is billed at the same time (i.e.; can't use with modifier 25).
