I have a patient that had diabetes that caused Neuropathy. The patient went through a weight loss surgery and their A1C is now only slightly elevated, not in the Diabetic range and on no meds, but the patient still has the neuropathy. I’m trying figure out how to code this, coding neither G62.9 or G63 seems appropriate to me, and I’m not seeing guidelines for coding Diabetes(E11.40) in such a case. What are your thoughts on a case like this?

Does the physician state the patient no longer has diabetes? You cannot assume no DM due to A1c levels:

Guidelines states:

The assignment of a diagnosis code is based on the provider’s diagnostic statement that the condition exists. The provider’s statement that the patient has a particular condition is sufficient. Code assignment is not based on clinical criteria used by the provider to establish the diagnosis.

G63

Provider was queried states DM is now history of since her bariatric surgery-can G63 still be used since the dr is stating she still has "diabetic" neuropathy?
Would I code G63 plus Z86.39 or only the G62.9 for the neuropathy? Thanks for your help :)
