If either one of these are billed out, what diagnosis code would be accurate to follow it?

What is documented in the note that prompted the use of these codes?

Actually I am just gathering information for the doctors that I work with. Blue Cross has put out a provider fact in June 2014 and they mention those codes in it. They will want to know what kind of dx code to put there if it was not obtained.

These are used for BMI reporting measures.
Body Mass Index (BMI) Claim Requirement ? Effective August 1, 2014
For each office visit, Blue Cross will require the BMI diagnosis code be included on the claim. It does not need to be included with a specific line item. If the BMI is not obtained or documented, use one of the following HCPCS* codes:
Blue Advantage? (PPO) ? New Claim Filing Requirements June 2014 2014-027

Not Documented/Patient Not Eligible for BMI Calculation: G8422
