any ideas on how to code this:

POSTOPERATIVE DIAGNOSIS:
Locally advanced rectal cancer with full-thickness invasion of
posterior vagina, final pathology pending.

PROCEDURE:
Posterior pelvic exenteration with perineal phase, omental
J-flap, vertical rectus abdominis myocutaneous flap
reconstruction by Plastic Surgery.
