Hello, when coding surgical pathology for a screening colonoscopy with colon polyps found, would you use Z12.11 plus the polyp code or just the polyp code? We have patients that state that per their insurance company, we should be billing the Z12.11 for a screening. EX: pt comes in for screening colonoscopy and a polyp of the cecum was removed. I would use the dx D12.0 for benign polyp of the cecum. Thanks for any input.
