To my understanding and per 3M logic 211.3/211.4 are an 'umbrella" codes for adenomatous (adenoma and adenoma like ) polyps - i.e adenoma/hyperplasia/lymphoid aggregates/fibroephitelial ect...

So my question is what does code 569.0 NOS cover? and why only in anal/rectal area and not the colon as well?

We use 569.0 for anal/rectal polyps not specified or found to be adenomatous.

ICD-9 for colon polyps leads you to 211.3, but do you use that for hyperplastic polyps too, even though they are non-neoplastic? We've always wondered about that.

That "Adenomatous" is not only adenoma as I mentioned but also adenoma-like polyps - and there is a different morphology code for adenoma and adenomatous...
