If a newborn is not circumsized in the hospital but a few days later in clinic is that still included in the global of the delivery? If not do we use a modifier?

Circumcision is never included in the global of maternity care and delivery. It is a separate procedure on the newborn and can be done either in the hospital prior to discharge, or in the office at a later date.

The only modifer required depends on if a regional block is used with 54150 which is Circumcision, using clamp or other device with regional dorsal penile or ring block. If the block is not used then you must add modifier 52 for reduced services. This is part of the instructions in the CPT book.
