I am looking for direction when coding a Left upper arm AVG with a Gore Hybrid. Any resources would be greatly appreciated.
 
Hello: We have not done any of the Gore Hybrid Graft Placement, as of yet, but I would google this and see what you can find, there is some info out there.. also you can call manufacturer reimbursement dept and they can often assist you.

Since this seems to be a nonautoghenous graft I would look at code 36830..Maybe with a mod 22??

When I get time I am going to look into this myself because I am sure that we will end of doing these...

Yes, this does help. I am considering the modifier 22. I did find a Z-health Q&A 3700 response that says to only code for the insertion of the new graft 36830.
