The OB clinic manager let me know today that we've started doing group OB visits ... always told after the fact. Looking online, it appears that these would be included in the global, but I wanted to make sure of this. I also saw where with some insurance companies (not stated which ones ... but to research individual insurances) to bill with an E&M with an add-on of 99078. I can see where this could be done if we do 1 to 3 visits and not the global. Has anyone out there had any experience with this? I'm in Oregon, so any Oregon Medicaid info would be hugely helpful. I'm asking the Midwives to document time in group, time individually, and to specifically state that it is a GROUP visit.

Any information anyone has would be appreciated.

I would also recommend keeping an eye on the upcoming maternity billing changes. Beginning January 1, 2027, global maternity billing will no longer be used.

Your practice will want to stay current on the guidance as details are finalized and make sure providers are prepared for any new documentation requirements. You’ll also want to clarify payer-specific billing instructions for patients whose maternity care spans both 2026 and 2027, since those cases may require special handling during the transition.
