Can you give an example of a situation where a facility would need to bill one of these modifiers? Facilities can be an originating site for telehealth when a patient is on their premises and the provider is off-site, and the hospital would then bill an originating site fee. But most facility services (e.g. radiology, infusions, OR procedures, etc.) can't be performed via telehealth so I can't think of a situation where one of these modifiers would apply to a facility bill.
