I have a question about the details of billing H0015 for Intensive Outpatient Substance Abuse. I understand that is it recommended as "at least 3 hours per day, and at least 3 days per week"...but what if the client only comes to 2 sessions that week and misses the 3rd? Should I be billing out the H0015 on those two days, or should the code be changed due to the 3x3 not being met for that week?

Did you ever receive an answer to this question? I am looking for the answer too.
 
Indiana Medicaid has very specific rules on IOP and if the 3 x 3 threshold is not met, they need to be billed as group/individual therapy. You might need to check payer rules.
