The patient received 8 doses of a drug in the outpatient setting over 2 days. One dose for the patient was 225 mg. One unit of this drug is 500 mg. Are the total mg added together to arrive at the number of units billed? Or are the number of doses used to bill one unit per dose?

Are there guidelines that specify if one unit per dose can be billed (8 units)? Or if 4 total units should be billed (225mg x 8 doses = 1800 mg)?

If someone has some experience with this - Thanks!
