If some payers (commercial) will cover an age appropriate preventive code, G0101/S0610/S0612, and Q0091 done on the same service date, I would like to see what ICD 10 would be appropriate. Since there are rules for first listed and some excludes 1 would the Z00.000 be the only appropriate way to code? Is it allowed to use Z00.000 on the preventive, then Z01.419 for example on G0101 since they are separate line items?
Also I am inquiring for our group that if billing for just the G0101 and Q0091 would you bill with DX Z01.41_ on first line then Z12.4 on the second? I interpret that Z01.41_ would be the only code to use for both lines due to the excludes 1 note. There are varying opinions based on having them assigned on different lines therefore not coded "together" which ties in to the first portion of my question too, with each line associating it's own DX. I feel the description of Z01.41_ states includes gyn exam with/without cervical smear so that would be all that is needed for both G0101 and Q0091.
