ACOG states that for Medicare preventive screening services, that if the patient has a hysterectomy, not to use the Diagnosis codes Z01.411 or Z01.419 (Encounter for routine GYN exam. They say to use Z12.72 (screening for malignant neoplasm of vagina) or Z12.79 (screening for malignant neoplasm of other genitourinary organs.)
Z12.72 has a description of "Vaginal pap smear status -post hysterectomy for non-malignant condition"...does that mean that the provider MUST do a vaginal pap smear to use this code? What if we are doing a visual examination, but not running any test? Would I still be able to use this code?
