Hi, one of our physicians did a hysteroscopy w D&C for a diagnosis
of dysfunctional uterine bleeding with a previous of an endometrial
ablation done in 2011-My question is what diagnosis code would this be ?

You could use the "personal history of..." codes in the V section of the ICD9 book; history of surgery, history of other gyn disorders.
