Hello,

I need some help regarding gyn surg coding.

I have 58145-82: Myomectomy Sngl/Mx (Sep Procedure)
then 58260-82: Vag Hyst
then 57250-82: Post Colporrhaphy Repr Repair

Ins comp paid for 58260-82 and 57250-51-82 and denied 58145-82 for incidental.

NOTE*: my provider was 1st ass during surg.

Q.: Can I still appeal this as NOT incidental and if yes should I use mod. 51

In the future it is best to code the more expensive CPT of the 2 procedures. I know it may seem wrong to report the myomectomy when the patient also had a hysterectomy but this pays more and is what is recommended.
