Wiki H&P performed with level 3 visit, raise level 4?

Good morning:
If our Orthopedic surgeon is doing a level 3 EM, and a decision for surgery is made and his PA performs the H&P at the same visit, can the MD raise his EM level to a level 4?
Thanks in advance for any input.:)
 
I forgot to mention these are both in our EMR system. The orthopedic surgeon wants to know if he has to mention the H&P is being done in his EM note by his PA? Thanks again. ;)
