Alrighty i have e&m vst that is technically a np but at the same vst he is doing his pre op can and should i do the np charge or do i have to use the h&p and/or would there be modifiers be involved if its a combo?
 
If this is an inpatient admit the charge the admit H&P and add the 57 modifier. If it is a new patient in the office setting then charge the new patient level with the 57 modifier. I am not sure what you mean by technically an np. Can you clarify?
 
"technically" meaning we have never seen this patient before and it is my understanding that the 57 would be used in cases where the operation would be in a 24 possibly 48 hour period?
The surgery is scheduled 4 days out
 
Then it is a new pt. What was this vsit for then I am a little confused. You have never seen the pt before, they come to your office for ?? and your physician evaluates them and decides that he will do surgery in four days? Is this the scenario? if so then it is a new patient visit.
