If a provider admits a patient from the office, can the admit be billed based on the documentation from the office? There is no H&P actually dictated, just the office notes, but if the documentation supports an H&P can that be billed instead or does it have to be dictated into the hospital system. Is there anything in writing to support this? Of note, the office note is scanned into the hospital system.
