We are in Arizona billing for a physical therapy group. Medicaid has changed its policy on # of visits allowed per year; its now 15 rehabilitative visits plus 15 habilitative with the correct modifiers. We found modifier SZ and have been denied for missing/invalid modifiers. What am I missing?!
