Can someone help me understand why metatarsal osteotomy 28306 is bundled with cheilectomy? I understand why they're bundled with bunionectomy, but 28289 doesn't involve resection of the medial eminence. Why does the CMS manual include 28289/28291 with bunionectomy code rules? Thanks!
 
One possible reason would be that smaller procedures performed in the same area as another procedure are not usually reported separately. The hallux rigidus procedures include work on the distal metatarsal and proximal phalanx. The medial eminence would be on the side of the distal metatarsal. All these procedures would be performed through the same surgical approach. To me that would be one possible reason for bundling the ostectomy for the medial eminence.
