Is Handwritten signature is acceptable in EHR charts ? or we can raise flag as missing provider signature.

Even if the handwritten signature is saved in an EHR system, there must be authentication that it was the provider who posted that signature to the note. CMS has indicated this authentication includes verbiage such as "Approved by", "Closed by", "Electronically signed by", etc and must include date and time of authentication.
