We have been billing the harvest of the tendon as either 25310 and 25312 and getting paid. Recently, UHC is saying this is incorrect. We have used 20924 before but that is when the tendon is completely severed and in most of our cases it is not. We cannot seem to come up with a better code that describes what our MD is doing. Here is what the op note states:
"Tendon harvest was accomplished by making a separate transverse incision in the forearm. This was done approximately 12 to 15 cm proximal to the volar wrist crease. Bluntly spreading the FCR tendon was identified and pulled into the wound and transected in its entirety with a 15 blade where I can see the muscle fibers inserting. This tendon was then pulled into the distal wound and left attached at its base. This completes the tendon harvest."
