Wiki HCC coding dx from lab, radiology and path?

I'm just checking as in normal coding we would pick up the dx from the lab, radiology and path, but am I correct that in HCC you don't pick up those dxs?

Can someone also tell me.....which documents in HCC coding you do pick the dx up?

I am not finding any resources for HCC coding.

HCC coding is still DX coding. If you are coding for the PCP your physician needs to docuemnt the findings of the Lab or Radiology provider. A coder should not code from these reports in the PCP setting.
