Wiki HCC Coding question

Hello,
I have a question for the HCC Coders out there. We are trying to determine the correct order for HCC coding. My co worker states she was taught that the reason for the visit should be the primary Dx (agreeable), but we are wondering if that is still the case when you are really trying to get all of the HCC codes on the claim? Do you still put the reason for the visit as primary? Any input is appreciated.

hi kelly This was always a challenge for me. do you have quality codes you add to the claim?

New to HCC coding, but the company I work for orders the codes based on their HCC value, not necessarily the reason for the visit. Hope that helps!
