Can anyone please tell me what are the 4 required documentation items in the record for a specific DOS.

I believe its the MEAT Monitor, evaluate, assess and treat. Maybe you already got the answer.

I'm not sure if you're looking for MEAT or PHI? Can you provide more info?

An E&M visit has it's required elements (see CPT book for details)
MEAT/TAMPER is required for each diagnosis. Only one element from MEAT/TAMPER is required as one shows proof the condition was dealt with.
PHI - protected health info includes the name, DOB, DOS which is required on each page of the progress note
Of course you also need the clinician name and credentials along with their signature and authentication
